Program Summary:
You’ve made a wise decision to enroll your practice in the Loyal Team™ Rewards program
from Loyal Patients, Inc™. This Agreement specifies how Loyal Patients, Inc.™ (LPI)
will provide its Loyal Team™ Rewards (LTR) program (Program) for your employees.
Under the Program, LPI will enable you to reward your team members with access to
brand name travel, entertainment and merchandise benefits to express your gratitude
for their loyalty and their successful job performance.
Providing Your Account Information:
You agree to upload certain data concerning your team members on the LPI "Register
Your Team" page of this website. You also agree to provide certain information about
your practice required by LPI in order to support the LTR program for your practice.
Billings and Your Right To Cancel:
- After entering your credit card number below, you will be billed the appropriate
amount (based on the number of team members you register for the LTR program) for
your first month of service and each month thereafter.
- There are no long-term contracts required. You may cancel your participation in
the Program at any time by calling LPI’s customer service number at 703-442-0079.
After cancellation you will not be billed for subsequent months.
- There is no set-up fee required to participate in the LTR program.
- You agree to pay the fees specified based on the number of team members you choose
to enroll in the Program (Note: number of team members includes any dentists enrolled
into the Program).
Your Payments Will Be As Follows:
Ongoing Monthly Program Fee: Once you submit your credit card for payment, you will
be billed for your first month’s participation in the Program. You will then be
billed for each subsequent month of Program participation at intervals of approximately
30 days. Each month, approximately five days before billing, you will receive a
Courtesy Advance Billing Notice to advise you of your upcoming billing.
Terms and Conditions:
You also agree to LPI’s Terms and Conditions of Use posted on the LPI website at
https://www.loyalteamrewards.com/TermsAndConditions.aspx.
Submission of this Agreement will constitute your acceptance of these obligations.
If there is a conflict between this Agreement and LPI’s Terms and Conditions of
Use, the terms of this Loyal Patients, Inc. / Team Rewards Agreement will control.
Refund Policy:
Services billed on credit card are paid for one month in advance and no cash refunds
are available for services provided in previous months or in the current month.
However, you will not be billed for subsequent months after 30-days prior written
cancellation. There are no cash refunds on cancellations of programs paid for via
LPI’s financing program; in this event, only a credit on other LPI products or services
will be provided for unused months, after the required 30-days prior written notice
of cancellation. Mailing fees are not refundable.
